How do I recover my user name?

To receive an email with a link you can use to recover your user name, enter your Last Name, First Name, Date of Birth, and Social Security Number.  Click the Submit button to complete the request.

What does the Cancel button do?

The Cancel button returns you to the Log In page. Any information you entered will not be saved.

What does clicking the HHS logo do?

Clicking the HHS logo in the upper left hand corner of the application allows you to return to the home page. Information not saved before clicking the logo will not be saved.
