How do I recover my user name?

To receive an email with a link you can use to recover your user name, enter your Last Name, First Name, Date of Birth, and Email Address.  Click the Submit button to complete the request.

What does clicking the Cancel button on the bottom of the page do?

The Cancel button will take you to the home page, but you will not be logged in.

What does clicking the Submit button on the bottom of the page do?

The Submit button will submit the information you have entered on the page and save it.  

What does clicking the HHS logo do?

Clicking the HHS logo in the upper left hand corner of the application allows you to return to the home page. Information not saved before clicking the logo will not be saved.
